Problem Statement Of The Client

The client faced difficulty in maintaining and managing vital patient data and wanted us to create a behavioral health integration with a Chronic Care Management (CCM) to address their challenge.

Lack Of Seamless Integration

The client found it difficult to integrate remote patient monitoring devices like blood pressure monitors, pulse oximeters, weighing scales, thermometers, and glucometers into their platform in real-time. This made it difficult for them to provide any type of consultation accordingly.

Physician-Patient Communication

The client wanted us to address the inability to connect with patients for routine appointments over secure video visits, difficulty in collecting patient vitals through remote patient monitoring, lack of video archives and auto-scribe capabilities with our solution.

Patient Experience

The client also wanted to address the challenge of limited options available for patients to receive timely care at a lower cost, lack of automatic reminders for patients to measure vitals, and increased emergency room visits and hospital stays.

We followed a three-step process to thoroughly understand the challenges being faced by the client before building a custom solution.

The second phase involved creating mockups of the platform. We used this time to define te platform’s features, functionalities, and aesthetics after getting a clarity on the platform’s requirements. We also sent these mockups to the client for their approval.
